The Palliative Care Social Worker participates in the activities of the interdisciplinary Palliative Care Team. The PC MSW participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care, works in collaboration with physicians, hospital, home health and hospice health care teams to provide Palliative Care services in patient, out-patient and in home settings to seriously ill patients and their families.







Required qualification:









  • Master's Degree in Social work or equivalent



  • 3 years Inpatient, Outpatient, or home-based Social work



  • Demonstrated evidence of experience in palliative care settings, such as hospice, i.e., ability to: a.Work independently as well as collaboratively on a small interdisciplinary team, b.Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care.









    Preferred qualification:









    • Working knowledge of research techniques, data collection, and policy, program and curriculum development



    • Experience with quality improvement methodology

      About the Team



      Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.







      HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
