health crisis in California. Illumination Foundation (IF) is a
growing non-profit organization dedicated towards disrupting the
cycle of homelessness by providing targeted, interdisciplinary
services in our recuperative care centers, emergency shelters,
housing services and children's and family programs. IF currently
has 13+ facilities with 22+ micro-communities scattered across
Orange County, Los Angeles County and the Inland Empire.Job
Description:Lead Care Manager is an interdisciplinary, high touch,
client centered role, which assumes primary responsibility for
coordinating members needs through a variety of engagement
modalities, requiring adherence to and application of
evidence-based practices, knowledge of client and service barriers
as well as social determinants of health, and deliverance and
coordination of services. Lead Care managers will function as a
community facing position wherein they will seek out populations of
focus and identified by providers and engage, assess, enroll, and
advocate for them on a routine basis. The pay range for this
position is $23-26 per hour, depending on experience.Client
Care:
Lead Care Managers will be
responsible for all members' needs, regardless of setting, such as
but not limited to:Implementation of care management
planIdentification of barriers to treatment and medication
management.Assist with navigation of physical health, behavioral
health and social services.Monitoring and supporting treatment
adherence.Assisting in attaining established goalsCoordination of
all appointments including accompaniment to the scheduled
appointment when necessary.Monitoring referrals and following up to
ensure services are accessed.Successfully facilitate transitions
among facilities.Verifying optimal communication among all service
providers.Documentation:
Lead Care Managers will be
responsible for appropriately navigating and documenting all
engagement frequency and content in Illumination Foundation's EMR
in accordance with standards of practice as required by
Illumination Foundation and associative contract entities. These
practices include:Incrementally documenting all aspects of a client
care plan, including goals met as well as goals
forthcoming.Documenting level of engagement such as frequency and
duration of encounters.Documenting evaluative client case details
which factor into decisions made to referral clients to alternative
resources.Documenting obtained client records, such as vitals
documents, insurance cards, SSI award letters, etc.Documenting
client disengagement as well as engagement.Documenting any other
such desirable reporting metrics as outlined and required by
contracting entitiesExpectations:
Knowledge and applied practice of
HIPAA parameters All company actions will be performed in
accordance with Illumination Foundation's guiding principles -Adept
levels or awareness and utilization of Illumination Foundation's
EMR systemProfessional and discreet interpersonal engagement with
all established and prospective clients as well as fellow care team
membersSufficient knowledge and utilization of peripheral services
which may be necessary to fully address client needs, as well as
the community support services being offered in the particular
county of operationContinued sensitivity to and awareness of
cultural and linguistic diversity -Honesty, transparency, and
fluidity in all client engagements and engagement attemptsAwareness
of Illumination Foundation models of care such as harm reduction,
and social determinants of healthAstute knowledge of, and diligent
effort being made on assembling and following up on comprehensive
assessments and care management plansFulfill Illumination
Foundation's Guiding Principles of Integrity, Humility, Sustainable
Compassion, Quest for Knowledge and Critical Thinking,
Client-Centered Care, Innovation, Advocacy and Stewardship and
Compliance.Preferred Experience/Minimum Qualifications:Required:
High School Diploma or equivalent. -2
years relevant experience.Basic computer skills, including the
ability to send and receive emails and summarize data in
spreadsheets. -Valid CA Driver's License and eligibility for
company vehicle insurance. -Preferred:
Bilingual in English and
Spanish.Proficiency in Google Suite (Mail, Docs, Sheets,
